Filmmaking Paperwork

Below is a collection of Frequently Used production forms, templates, and charts used for SCA film projects.

Students will be asked to submit these forms electronically through the form itself or by uploading it to their project's Smartsheet Project Dynamic View.

If you have questions or need help filling out any forms, don’t hesitate to email the Production Office at [email protected].

Project Information Form

Planning to do a shoot that is beyond the Anytime Project criteria (check out equipment, reserve a stage at Cinespace, or rent gear or locations for your film)? This is the form for you!

Once you submit it, the SCA Production Office will set up your project and guide you through the next steps. We’ll reach out to gather more details and help get your production rolling.

Submit your form at least seven business days in advance.

While you wait, take a look at the rest of the website and complete the SCA Production Training on D2L – a one-time requirement each academic year to participate in any SCA film project.

SCA General Release Form & Assumption of Risk

Everyone on your cast and crew – students, non-students, and minors – needs to sign this University-required General Release form, prior to entering any film shoot location as cast or crew.

Cast and Crew List Template (To Be Submitted to Production Office)

Please use this Cast and Crew List Template for your project.

Be sure to read and follow the instructions on the top of the template or your list may be rejected.

Cinespace Cast and Crew List Instructions – Stage Use & Entry

This template is ONLY for Cinespace access and is separate from the general Cast & Crew Template required by the Production Office. Use it if your student film project needs Cast & Crew access to Cinespace for equipment pick-up, drop-off, or stage reservations.

Follow the instructions carefully and grab the template via the QR code. You must submit a .csv file using the exact template columns – we can’t accept lists that don’t match the template.

Project Change Form

Need to change your production dates? Whether it’s a reshoot or adding extra days, this is the form to use.

When you’re done, just upload it to your Smartsheet Dynamic View so we can review and approve it.

Stunt Evaluation Chart

A Stunt Coordinator is the go-to person for all stunts on set. They plan the action, cast stunt performers, and make sure everything is done safely.

Depending on how much action your project has, you might need a stunt coordinator.

Intimacy Evaluation Chart

An Intimacy Coordinator makes sure intimate scenes are done safely, respectfully, and with everyone’s consent. They choreograph the scenes, support performers, and help everyone communicate clearly.

Depending on how intimate your project is, you might need an intimacy coordinator.
